In-Office Sessions

  • My office is between Belton and Nolanville, TX, close to the Lake Belton region. You will be given the exact address after your consultation call and in your confirmation emails.

    Please try to arrive 5-10 minutes early and stay in your car until the previous client leaves or I signal you to enter via text.

    Your session will take place in a private treatment room to allow for full privacy.

  • Cost: $150

    Time: 1 hour

    Doctor’s Script required: 2 weeks after evaluation

    Your initial evaluation will last a total of 60 minutes and is always one-on-one. The first session allows for enough time to hear your story and history, assess for a root cause, and begin treatment. You can request an additional 30 minutes ahead of time for a slight upcharge if needed.

  • Cost: $150

    Time: 1 hour

    Due to the follow up sessions requiring less subjective information, we have more time for treatment. These sessions are always one-on-one so we can focus solely on you!

  • Cost: $150

    No script required!

    These sessions focus on your maintenance of progress or goals not related to physical therapy.

  • *** At least 1 parent or guardian must be present for all sessions involving minors.

    Evaluation/follow up 1 hour: $150

    Wellness evaluation 1 hour: $150

    Clothing: Loose shorts and shirt preferably with minimal sleeves to allow access to shoulders and hips.

    I will have toys but please bring some of your own toys, snacks, and other means to help occupy your child as they get used to the new environment.

    A driver’s license or ID must be provided by the parent at the initial evaluation and a consent form will be filled out to allow for other parents or family members to bring your child in to their sessions.

  • Please arrive wearing loose fitting clothing. Loose shorts, braziers, and tank tops can be changed into in the room if needed to allow for access to your shoulders and hips.

    Scripts can be sent by your physician, certified nurse midwife, chiropractor, or nurse practitioner to topliffdpt@gmail.com or faxed to (254) 304-0164.

  • A phone consultation allows you to ask questions about physical therapy, see if Selah will meet your needs, and discuss pricing and scheduling.

  • PT Evaluation: $150

  • Selah is not in-network with any insurances but can provide superbills that you can send to your insurance for reimbursement. I will work my hardest to ensure that your insurance will hep your cover these cost by making my notes as insurance friendly as possible.

  • Selah PT is not a provider for Medicare and therefore cannot provide physical therapy to Medicare patients.

    Medicare patients CAN receive Wellness sessions at Selah PT and this works very well with receiving regular PT at a Medicare providing clinic as Wellness sessions are typically only 1 time monthly.

  • Absolutely! If we decide that Selah would be a good fit for you, we can set up your first session over the phone right away!

Special notes and policies:

Cancellation Policy: 24 hours notice are required for any cancellations of your appointments. Last minute cancellations, no shows, or changes to your appointment will incur a fee that must be paid prior to receiving any additional sessions from Selah Physical Therapy. 

Full cancellation Policy:

Late cancel or reschedule (less than 24 hours notice): $50 fee

Second late cancel or reschedule: $100 fee

Third late cancel or reschedule: $135 fee

No show (failing to show up despite having received confirmation emails and texts without any advanced notice from you to Selah PT via email, text, or call): $135 fee
