In the state of Texas, you can receive physical therapy for 30 days after the start of treatment. Within 30 days from the start of care (initial in-person evaluation) a physical therapy referral or script will be required from a physician (MD or DO), chiropractor (DC), nurse practitioner (NP), dentist (DDS), or certified nurse midwife (CNM),

Wellness Sessions DO NOT require a script as they focus solely on goals outside of PT and involve Wellness related treatments only.

Selah Physical Therapy is a cash pay provider only and can provide Superbills for you to give to your insurance provider to request reimbursement.

Selah Physical Therapy is not a Medicare provider. Medicare clients can only receive Wellness Sessions.

Special notes and policies:

Cancellation Policy: 24 hours notice are required for any cancellations of your appointments. Last minute cancellations, no shows, or changes to your appointment will incur a fee that must be paid prior to receiving any additional sessions from Selah Physical Therapy. 

Full cancellation Policy:

Late cancel or reschedule (less than 24 hours notice): $35 fee

Second late cancel or reschedule: $70 fee

No show (failing to show up despite having received confirmation emails and texts without any advanced notice from you to Selah PT via email, text, or call): $125 fee
